This privacy statement explains how CVD handles the collection and use of personal data. This primarily concerns data from staff, clients, applicants, students, and users of the CVD website. CVD processes data in accordance with the GDPR. <\/p>\n\n\n\n

Personal data that are processed<\/h3>\n\n\n\n

User website<\/h4>\n\n\n\n

CVD may process personal data when someone uses the CVD website and\/or when they provide it to CVD when completing a contact form on the website. When using the website, the following personal data may be processed: name, (email and other) address information, telephone number, IP address, and cookie ID. <\/p>\n\n\n\n

Support<\/h4>\n\n\n\n

When CVD provides support, CVD also processes the Citizen Service Number (BSN), the ELV application, the Long-Term Care Act (Wlz) application, the Social Support Act (Wmo) application, income data and any other financial data, information about daytime activities used, information about housing, family situation, judicial information, educational history, and (special) personal data about well-being and health. We process this information to account for the care provided, determine the personal contribution, process the application for Long-Term Care Act (Wlz), or assess the insurance situation. All of this is in fulfillment of the agreement between CVD and the client. <\/p>\n\n\n\n

We retain a Long-Term Care Act (Wlz) care plan or referral letter for a medical specialist or general practitioner for up to 20 years due to the medical retention obligation. We retain other information related to the Long-Term Care Act (Wlz) and Social Support Act (Wmo) for at least 15 years and then for a maximum of 2 years after we last provided services to the client. <\/p>\n\n\n\n

Applicants<\/h4>\n\n\n\n

Additional personal data is collected from applicants in preparation for the potential conclusion of an employment contract. This includes employment history, educational history, the remainder of the cover letter and CV, and any screening. We retain application data for a maximum of four weeks after the application process has closed, or for one year with the applicant’s consent. <\/p>\n\n\n\n

Interested parties<\/h4>\n\n\n\n

Data may also be collected because interested parties leave it on the website to keep them informed of various CVD activities. We retain this data for two years after our last contact. <\/p>\n\n\n\n

Students<\/h4>\n\n\n\n

Data may be processed for the purpose of recruiting students. We retain this data for a maximum of one year after completion of the course, or for up to seven years due to tax retention requirements. <\/p>\n\n\n\n

Origin information<\/h4>\n\n\n\n

The data is provided by the data subjects themselves and may come from third-party source systems, such as municipalities (see www.cvd.nl\/over-ons\/onze-partners), CIZ (www.ciz.nl), Tax Authorities (www.belastingdienst.nl) or PFZW (www.pfzw.nl).<\/p>\n\n\n\n

Use of data<\/h3>\n\n\n\n

The data is used to properly perform the associated tasks and legal obligations as part of the business operations of a social service provider and employer. Student data is used specifically for providing courses and to contact students (by phone, email, or mail). Personal data may also be used to execute an agreement concluded with the client for the purpose of providing support. Furthermore, data is used to collect personal contributions under the Social Support Act (Wmo), handle complaints and disputes, and perform audits. <\/p>\n\n\n\n

Data retention period<\/h3>\n\n\n\n

CVD does not retain personal data for longer than is strictly necessary to achieve the purposes for which the data is collected and processed, unless this is necessary for historical, statistical, or research purposes, or pursuant to a legal requirement. The retention periods to be applied by CVD are established. <\/p>\n\n\n\n

Sharing data with third parties<\/h3>\n\n\n\n

Third parties are only provided with personal data in specific cases. For example, when we use processors (suppliers who process personal data for us on our behalf and in accordance with our rules). We also share data with the Municipality, the CAK (Central Administration Office), or other authorized government agencies, including in the context of executing an agreement with a client or providing legal support measures. Furthermore, we only share data if the data subject has given permission (for example, with specialists, health insurers, or partners). Furthermore, we only share data with our subcontractors for day care and with companies providing domestic help for our clients when necessary. Data that cannot be traced back to individuals may be provided to third parties for statistical, historical, or scientific purposes. <\/p>\n\n\n\n

Access to data<\/h3>\n\n\n\n

The collected data is not accessible to everyone. Only employees and individuals working on behalf of CVD have access to the data if this is necessary for the performance of their duties. Through robust security, those authorized to access the data are authorized. All employees with access are bound by confidentiality under their employment contracts. <\/p>\n\n\n\n

Use of data users website CVD<\/h3>\n\n\n\n

CVD’s website tracks general visitor data, including the IP address, a visitor’s cookie ID, the time of the request, and data sent by the browser. This data is used to analyze visitor and click behavior on the website. CVD uses this information to improve the website’s performance. <\/p>\n\n\n\n

Rights of data subjects<\/h3>\n\n\n\n

Individuals whose data is processed by CVD have the right to access, correct, or delete their personal data. They also have the right to object to certain processing, to have processing restricted, or to receive or have the personal data processed by CVD transferred in a readable format. They can submit a request to CVD’s Data Protection Officer (DPO). The DPO will respond to the request as soon as possible, but in any case within four weeks. Please ensure that you clearly identify yourself in your request so that the DPO can verify your identity. <\/p>\n\n\n\n

CVD takes the protection of personal data seriously and implements appropriate technical and organizational measures to prevent misuse, loss, unauthorized access, unwanted disclosure, and unauthorized modification. Appropriate security measures are in accordance with the NEN 7510 standard for information security in healthcare. CVD is certified for this. Furthermore, the CVD website uses an SSL Certificate to ensure that personal data does not fall into the wrong hands.
